Despite being one of the <a href=\"https:\/\/academic.oup.com\/jcem\/article\/108\/10\/2447\/7242360\">most common endocrine disorders in women of reproductive age<\/a>, many never get diagnosed at all.<\/p>\n\n\n\n<p>Part of the problem is the name. Despite being called Polycystic Ovary Syndrome, PCOS isn&#8217;t really about cysts. It&#8217;s a metabolic and hormonal condition that may involve the ovaries. That&#8217;s why in May 2026, PCOS was <a href=\"https:\/\/www.notion.so\/PCOS-explained-and-why-metabolism-plays-a-huge-role-35f755317a9180009559fbef15da101f?pvs=21\">renamed PMOS \u2014 Polyendocrine Metabolic Ovarian Syndrome<\/a>.<\/p>\n\n\n\n<p>That shift in framing \u2014 from an ovarian condition to a metabolic one \u2014 is what changes thinking and approach. It&#8217;s also what makes PCOS unusually responsive to the things you can measure and change.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-what-is-pcos\">What is PCOS?<\/h2>\n\n\n\n<p>PCOS presents differently in different women.<\/p>\n\n\n\n<p><strong>Fertility problems<\/strong>: Irregular, infrequent, or absent periods. Trouble getting pregnant.<\/p>\n\n\n\n<p><strong>Skin and hair<\/strong>: Excess hair on the face, chest, or stomach. Facial and chest acne. But this framing can guide users to lifestyle interventions that can support treatment.<\/p>\n\n\n\n<p>Insulin tells your cells to absorb glucose. When they stop listening, the pancreas pumps out more insulin to compensate. That <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC3277302\/\">elevated insulin directly pushes the ovaries to make more androgens<\/a>. This fuels hormonal disruption, irregular cycles, skin and hair symptoms, and difficulty losing weight.<\/p>\n\n\n\n<p>Insulin resistance affects <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC3277302\/\">around 65\u201370% of women with PCOS<\/a>. A common misconception is that you need to be overweight or pre-diabetic for it to be the driver, but roughly <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC3277302\/\">20\u201325% of insulin resistant women with PCOS were of a healthy weight.<\/a><\/p>\n\n\n\n<p>Insulin resistance isn&#8217;t a fixed state. It responds to food, exercise, sleep, and stress on a timescale of days and weeks, not years.<\/p>\n\n\n\n<p>These problems feed each other. Stress raises androgens. High insulin raises androgens. High androgens disrupt the signals from the brain that should be triggering ovulation \u2014 so periods become irregular, which is itself a sign the cycle isn&#8217;t working. And <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4334071\/\">inflammation quietly makes insulin resistance worse<\/a>, starting the loop again.<\/p>\n\n\n\n<p>The good news: because everything is connected, you don&#8217;t have to fix everything.
